Stock Donation

Your support of VIP Services, Inc. through the gift of stock is greatly appreciated. You may elect to have your stock transferred electronically or you may wish to transfer stock certificates directly. In either case, you or your broker will need the following information:

• Name and complete address of donor

• Name and number of securities being transferred

• Name of brokerage house or other entity handling the transfer
